Genome analysis of an OXA-48-producing carbapenem- and colistin-resistant Klebsiella pneumoniae sequence type 11 clone isolated from an inpatient

Colistin is one of the last-resort therapeutic antibiotics used to control carbapenem-resistant Klebsiella pneumoniae. However, emergence colistin-resistant strains with high mortality and morbidity has become a new challenge. Therefore, it crucial characterize genotypic features carbapenem- K. pneumoniae (CCKP) isolates implement public health intervention strategies. In this study, we analyzed whole genome an OXA-48-producing CCKP strain for molecular characterization antimicrobial resistance (AMR) as well comparative plasmid analysis. The whole-genome analysis showed that IRAN-43 possessed 140 contigs 5,585,691 bp length, GC content 56.9% N50 score 200,728. Multi-locus sequence typing (MLST) belonged type (ST) 11. Single amino acid substitutions associated colistin were found in PhoP (D150G), PmrB (R256G) RamA (I25T). addition, two plasmids harboring qnrS1, tetR, tetB (plasmid-1) blaOXA-48 (plasmid-2) discovered. No plasmid-mediated (mcr) genes detected. Integrative conjugative element (ICEKp)4 variant was detected cps locus length 22,156 encoding 17 open reading frames (ORFs) which capsular (KL)15-1. been increasingly reporting from Iran. High frequency antibiotic among ST11 suggests stricter measures are needed prevent spread clone hospital settings.
